Puckering (that wrinkled, tunneled look around the stitching) is the most common embroidery complaint, and it almost always comes down to stabilizer and hooping. Here's the checklist I run.

Start by matching a proper cutaway to the fabric and hooping tight — that fixes the large majority of puckering. Post a photo (front and back of the sew-out) on the thread and an experienced digitizer can usually spot the rest.
